Managing Diabetes, Then Told of Cancer - NYTimes.com

Nine years ago, Brenda Gray, a former schoolteacher in North Carolina, discovered she had Type 2 diabetes.

Since then, she has learned to manage the disease, diligently taking her medicine and keeping tabs on her blood sugar. But in September, she was told she had skin cancer, and her diabetes spun out of control.

Ms. Gray started an aggressive course of treatment that included radiation therapy. But the treatments weakened her and destroyed her appetite. Unable to eat, she developed dangerously low blood-sugar levels, and about two months ago, Ms. Gray?s daughter had to rush her to a hospital.

?She found me in bed shaking and sweating,? said Ms. Gray, who is 62 and lives in Durham. ?When I got to the hospital, they couldn?t understand how I was still standing.?

Cancer and diabetes are two of the leading killers in America. Each can be a devastating diagnosis in its own right, but researchers are finding that the two often occur together. By some estimates, as many as one in five cancer patients also has diabetes.

In a recent joint report, the American Cancer Society and the American Diabetes Association noted that people with Type 2 diabetes have an increased risk of developing cancers of the liver, pancreas, colon and bladder. Researchers with the National Cancer Institute released a similar report last year, which found greater rates of cancer among diabetics, as well as an elevated risk of dying from cancer.

Experts say it is clear from accumulating clinical data that the two share some biological links. The problem results from simple demographics as well: with the rapid rise in Type 2 diabetes and a growing population of cancer survivors, the two diseases are coinciding more frequently in older patients.

?We are going to see a lot greater numbers of people with both diseases,? said Edward Giovannucci, a professor of nutrition and epidemiology at the Harvard School of Public Health and an author of the joint report. ?By some estimates, the link between diabetes and cancer may quantitatively become even more important than the link between smoking and cancer.?

Already, oncologists say, it is not uncommon to encounter patients struggling to balance cancer treatments with insulin shots and diabetes drugs. Because cancer is generally seen as the more lethal of the two diseases, patients often make it the priority.

?Although cancer is no longer generally a death sentence, for many patients, they see it as that no matter what you say,? said Dr. June McKoy, a geriatric oncologist at the Northwestern University Feinberg School of Medicine. ?Suddenly, they put their diabetes on the back burner, and they focus on the cancer.? But high blood sugar can damage kidneys and blood vessels, strain the immune system and worsen cancer prognosis.

Researchers say that the link between the two diseases is complex and driven by many factors. Typically, though, it is diabetes that sets the stage for cancer. ?Most cancers don?t cause diabetes,? said Dr. Pankaj Shah, an endocrinologist at the Mayo Clinic in Rochester, Minn. ?Mostly diabetes increases the risk of cancer.?

Type 2 diabetes is often preceded by chronically high insulin levels and high blood sugar, fertile conditions for cancer. Insulin is known to fuel cell growth, and cancer cells consume glucose out of proportion to other nutrients. The two diseases share many risk factors as well, including obesity, poor diet, physical inactivity, smoking and advanced age.

Another factor that complicates the relationship is the treatments given to patients. Diabetes drugs can have an impact on cancer prognosis and vice versa. Type 2 diabetics treated with the drug metformin, for example, develop cancer less frequently than diabetics given other medications. A number of clinical trials are now under way to see how well the drug performs as a cancer treatment.

Drugs used against cancer, on the other hand, tend to worsen diabetes.?Chemotherapy can wreak havoc on blood sugar levels, and glucocorticoids, which are widely prescribed to alleviate nausea in cancer patients, promote insulin resistance, said Dr. Lorraine L. Lipscombe of Women?s College Hospital in Toronto.

Dr. Lipscombe was the lead author of a large study last month that found that breast cancer survivors were 20 percent more likely to receive a diabetes diagnosis than other women. The study found evidence that glucocorticoids and chemotherapy may hasten the onset of diabetes.

?They don?t cause diabetes in everyone, but they can bring out or unmask it in people who might already be vulnerable,? she said.

For diabetics who are used to tightly monitoring their blood sugar levels, the impact of cancer drugs can be alarming. Rigoberto Cortes, 71, a former metal worker in Chicago, has had Type 2 diabetes for over two decades. A year ago, he was told he had Stage 3 colon cancer.

?When I started chemotherapy, my sugar level was going way up and way down like never before,? he said. ?I kept asking my oncologist what I should do.?

Mr. Cortes said his oncologist was not very concerned by the blood sugar swings. He eventually got a second opinion and switched doctors. He also lost weight and changed his eating habits, which helped minimize his blood sugar swings.

Although every case is different, the general strategy in treating such patients should be to get the cancer under control first, said Dr. Shah at the Mayo Clinic.

?Diabetes treatment essentially is given to prevent long-term complications,? he added.

At some hospitals, oncologists may take responsibility for managing blood sugar and other diabetes concerns in their cancer patients. But ideally, treatments should be coordinated by a team that includes a certified diabetes educator.

?They go over diet with the patient, review their medication, review their insulin,? said Dr. McKoy of Northwestern. ?They can play a big role.?

For a diabetic trying to navigate the world of cancer, or a cancer patient navigating the world of diabetes, such interventions can be crucial. In a study published in October, Dr. McKoy and her colleagues looked at several years of health records for over 200,000 people with Type II diabetes who developed cancer.

Those who underwent a diabetes counseling session after their cancer diagnosis ? consisting of two sessions a week for four to six weeks ? were more likely to receives tests of hemoglobin A1c levels, a barometer of how well blood sugar has been controlled over time, and to take care of their blood sugar levels. As a result, they had fewer emergency room visits, fewer hospital admissions and lower health care costs.

Ms. Gray, the former schoolteacher in Durham, learned this firsthand. After her recent emergency, she worked with a diabetes educator at Duke University Hospital. Ms. Gray learned tips and strategies to balance the two diseases, including ways to keep her blood sugar normal when cancer treatments ruin her appetite.

?I came into the hospital and they got me back on track,? she said. ?I was just so focused on the cancer. It changed everything. But I?ve learned how to face this.?

UFC 155: Cain Velasquez vs. Junior dos Santos full fight video highlights

Complete video highlights of Cain Velasquez' triumphant rematch with Junior dos Santos, the man who had taken his title last year. Velasquez dominated JDS for five brutal rounds following a jaw-breaking right hand in the first round that bounced dos Santos off his feet.

Tonight at UFC 155, Cain Velasquez reclaimed his Heavyweight title with a brutal, relentless five round drubbing of Junior dos Santos, the man who took his title last year at the first UFC on Fox TV.

Here's what our own Dallas Winston had to say in his live blog of the fight about the critical first round:

R1: Cain comes out pressuring with a 1-2 and then ducks under for a body lock takedown. JDS reverses it, like a boss, and almost takes Cain down. Cain is a little predictable on his next two attempts: he's just unsuccessful on the first but almost walks into a jackhammer on the second. JDS plunges a deep jab and sits down on a few hooks to back Cain off. Cain slings out a series of jabs that find the mark but he fails badly on the single.

JDS cracks him with the same right hand that put him away the first time, but Cain takes it in stride. Cain crowds him up on the fence but JDS sneaks out when he releases his grip to dirty box. Cain blisters a right hand through JDS' guard that drops him on his can. History is made. Cain is in top form and swarming JDS in classic fashion with positional advances and rapid-fire punches.

Barbara Greenberg: Do We Really Care About the Mental Health of ...

Listen, I know that the title of this article is very provocative. I also know that it is a very strange question coming from a clinical psychologist who specializes in the treatment of adolescents. Teenagers walk in and out of my office all week long and yes, it is their caring and devoted parents who bring them to their initial sessions because they want them to feel better, to feel peaceful, to feel joy and to feel anything but emotional pain. On the other hand, it is not until teens demonstrate clear symptoms of depression, anxiety and any of a number of other difficulties that interfere with academic and social functioning that a mental health expert is consulted. That leads me to yet another provocative question: Are we more concerned about our teens' mental health or their academic and social performance -- with an emphasis on performance? This is a question that I'd like all parents of teens to mull over. It is also not an accusation, but we must get to some sort of consensus about the answer.

I have yet another related issue that has been troubling me for years. Here it is in its raw form. We take our kids to their annual physician appointments to make sure that they are growing at the proper rate and that all of their other physical systems are intact. We check to see if they need any sort of tune up, if they need any of their oils changed and if their engines are running well. Sorry to compare your teenagers to cars, but the annual pediatrician or internist visit is generally about the physical and not the mental functioning of the teen -- sort of like how we think about how our cars are running.

I have a novel proposal which I believe is especially important especially in light of the recent tragic school shooting. I propose that all of our kids get an annual mental health assessment starting from the early years to the teen years and then also maybe every year following. Perhaps the stigma associated with mental health assessments would be diminished if mental health assessments were routine. Listen, I know that in the majority of annual physicals teens are not opening up about mental health issues. I've been to conferences about this and I've spoken to hundreds of kids and plenty of physicians. Unless it is a clear mental health problem that manifests itself physically like anorexia nervosa, a tic disorder, failure to thrive or some other physical health issue, it is unlikely to be identified. Even if the issue does manifest itself in a physical or somatic manner, as we like to say in the business, it may still go unnoticed. I say this with all due respect for physicians who generally go into the field with the best of intentions to heal people. I know that they are tight on time, that their training may have taught them to focus on the physical and that patients may not be providing necessary information.

Perhaps these annual mental health assessments that I am proposing need to be done outside of the physician's office and in the office of a mental health professional. We cannot, however, simply rely on the self-report of the child or teen, because self-report is often very inaccurate.

We need to start a movement where annual mental health assessments are required.
I would be more than delighted to be a part of this movement. Why does a mental health issue need to be immobilizing before it is dealt with?

Comedian Katt Williams arrested in LA

LOS ANGELES (AP) ? Katt Williams, the comedian who has repeatedly found himself on the wrong side of the law, is out on bail after being arrested in Los Angeles on suspicion of child endangerment and possession of a stolen gun.

Police Officer Norma Eisenman says Williams was taken into custody Friday after the LA County Department of Children and Family Services did a welfare check at his home. Authorities found more than one firearm, one of which had been reported stolen.

Eisenman says the DCFS did not specify how many children lived at the home or whether they were removed.

The 41-year-old was arrested this month on a felony warrant related to a police chase. In November, he was accused of hitting a man on the head with a bottle during a fight.

Last-minute fiscal cliff talks in Senate

WASHINGTON (AP) -- Senate leaders groped for a last-minute compromise Saturday to avoid middle-class tax increases and possibly prevent deep spending cuts at the dawn of the new year as President Barack Obama warned that failure could mean a "self-inflicted wound to the economy."

Obama chastised lawmakers in his weekly radio and Internet address for waiting until the last minute to try and avoid a "fiscal cliff," yet said there was still time for an agreement. "We cannot let Washington politics get in the way of America's progress," he said as the hurry-up negotiations unfolded.

Senate Republicans said they were ready to compromise. "Divided government is a good time to solve hard problems_and in the next few days, leaders in Washington have an important responsibility to work together and do just that," said Sen. Roy Blunt of Missouri, delivering his party's weekly address.

Even so, there was no guarantee of success.

In a blunt challenge to Republicans, Obama said that barring a bipartisan agreement, he expected both houses to vote on his own proposal to block tax increases on all but the wealthy and simultaneously preserve expiring unemployment benefits.

Political calculations mattered as much as deep-seated differences over the issues, as divided government struggled with its first big challenge since the November elections.

Speaker John Boehner remained at arms-length, juggling a desire to avoid the fiscal cliff with his goal of winning another new term as speaker when a new Congress convenes next Thursday. Any compromise legislation is certain to include higher tax rates on the wealthy, and the House GOP rank and file rejected the idea when Boehner presented it as part of a final attempt to strike a more sweeping agreement with Obama.

Yet lawmakers have until the new Congress convenes to pass any compromise, and even the calendar mattered. Democrats said they had been told House Republicans might reject a deal until after Jan. 1, to avoid a vote to raise taxes before they had technically gone up and then vote to cut taxes after they had risen.

Nor was any taxpayer likely to feel any adverse impact if legislation is signed and passed into law in the first two or three days of 2013 instead of the final hours of 2012.

Gone was the talk of a grand bargain of spending cuts and additional tax revenue in which the two parties would agree to slash deficits by trillions of dollars over a decade.

Now negotiators had a more cramped goal of preventing additional damage to the economy in the form of higher taxes across the board ? with some families facing increases measured in the thousands of dollars ? as well as cuts aimed at the Pentagon and hundreds of domestic programs.

Republicans said they were willing to bow to Obama's call for higher taxes on the wealthy as part of a deal to prevent them from rising on those less well-off.

Democrats said Obama was sticking to his campaign call for tax increases above $250,000 in annual income, even though he said in recent negotiations he said he could accept $400,000.

There were indications from Republicans that estate taxes might hold more significance for them than the possibility of higher rates on income.

One senior Republican, Sen. Jon Kyl of Arizona, said late Friday he was "totally dead set" against Obama's estate tax proposal, and as if to reinforce the point, Blunt mentioned the issue before any other in his broadcast remarks. "Small businesses and farm families don't know how to deal with the unfair death tax_a tax that the president and congressional leaders have threatened to expand to include even more family farms and even more small businesses," he said.

Officials said any compromise was likely to ease the impact of the alternative minimum tax, originally designed to make sure that millionaires did not escape taxation. If left unchanged, it could hit an estimated 28 million households for the first time in 2013, with an average increase of more than $3,000.

Taxes on dividends and capital gains are also involved in the talks, as well as a series of breaks for businesses and others due to expire at the first of the year.

Obama and congressional Democrats are insisting on an extension of long-term unemployment benefits that are expiring for about 2 million jobless individuals.

Leaders in both parties also hope to prevent a 27 percent fee cut from taking effect on Jan. 1 for doctors who treat Medicare patients.

There was also discussion of a short-term extension of expiring farm programs, in part to prevent a spike in milk prices at the first of the year. It wasn't clear if that was a parallel effort to the cliff talks or had become wrapped into them.

Across-the-board spending cuts that comprise part of the cliff were a different matter.

Republicans say Boehner will insist that they will begin to take effect unless negotiators agreed to offset them with specified savings elsewhere.

That would set the stage for the next round of brinkmanship ? a struggle over Republican calls for savings from Medicare, Medicaid and other federal benefit programs.

The Treasury's ability to borrow is expected to expire in late winter or early spring, and without an increase in the $16.4 trillion limit, the government would face its first-ever default. Republicans have said they will use administration requests for an extension as leverage to win cuts in spending.

Ironically, it was just such a maneuver more than a year ago that set the stage for the current crisis talks over the fiscal cliff.

2012 Year in tech: A timeline

DNP 2012 Year in tech A Timeline

By Billy Steele, Sarah Silbert and Christopher Trout

While planet Earth has yet to meet its demise, the end of 2012 is nigh. It was a year of lengthy legal battles and shifting power dynamics in the tech industry. It brought with it great advancements and great failures -- and, for some, the promise of the end of days. We've combed our archives to bring you just a few of the stories that made the biggest impact on our reporting this year. Herewith, an abridged look back at the year that was.

Egypt's Mubarak to stay in army hospital as health worsens

CAIRO (Reuters) - Egypt's deposed leader Hosni Mubarak, who is serving a life sentence for his role in killing protesters during a 2011 revolt, will stay in an army hospital for at least two weeks after his health deteriorated, his lawyer said on Friday.

The state of Mubarak's already fragile health has been the subject of intense speculation in Egypt and he has spent much of the time before and after his trial in various hospitals.

On Thursday evening, the 84-year-old former leader was transferred to an army hospital from his prison clinic after fracturing a rib in a recent fall.

"He will stay in the hospital for about 15 days," Mohamed Abdel Razek, his lawyer, told Reuters.

"The president's health is stable, thank God. He underwent X-rays on his body and now he will get proper treatment in the hospital for all his bone problems he has been suffering from."

Mubarak was toppled after 30 years in power and sentenced to life in prison in June this year. He was admitted to a prison hospital that same month following what security officials described at the time as a "health crisis".

During his trial, Mubarak, the first Arab ruler to be brought to court by his own people, was routinely wheeled into the cage used for defendants in Egyptian courts on a hospital gurney.

He has not spoken publicly about the events that followed his downfall, saying almost nothing at his trial appearances beyond confirming his presence and denying the charges against him.

Mubarak's legal team had been pressing to have him moved from the prison hospital to a better-equipped facility permanently, saying he was not receiving adequate treatment.

But there has been some confusion over the exact nature of his ailments, with state media reporting a variety of illnesses ranging from shortage of breath to heart attacks and comas.

5 issues facing small businesses in 2013

40 min.

In 2013, small business owners will contend with many of the same issues that made it hard to run their companies over the last 12 months.

They're also heading into the new year with a lot of uncertainty. It's unlikely that negotiations in Congress will resolve all of lawmakers' disagreements over tax and budget issues that affect small businesses. And there are still many questions about the implications of the health care law for small companies.

That points to continued caution ? and perhaps slow hiring ? among the nation's small companies.

"Uncertainty is the bane of every small business," says Scott Shane, a professor of entrepreneurship at Case Western Reserve University's Weatherhead School of Management in Cleveland. "Their only rational response is to pull in their horns and slow down."

Small businesses aren't likely to get much encouragement from the economy. It's expected to grow by no more than 3 percent in 2013, according to the Federal Reserve. That's a moderate pace, better than the 1.7 percent that the economy grew during the first three quarters of 2012. But it's also far from robust.

Here's a look at some of the issues facing small businesses in the coming year:

Taxes
Lawmakers are still haggling over what's called the fiscal cliff, the combination of billions of dollars in tax increases and budget cuts. Even if Congress reaches an agreement, small business owners won't have the certainty they need, according to Todd McCracken, president of the National Small Business Association, a group that lobbies on behalf of small companies.

"It almost surely won't be comprehensive enough that we won't be revisiting it next year," McCracken says. He's concerned that there'll be another fiscal cliff in six months ? which would mean more negotiations and more uncertainty.

Many small business owners are worried about their personal tax rates. Sole proprietors, partners and owners of what are called S corporations, all report the income from their businesses on their individual Form 1040 returns. That means their companies are in effect taxed at personal rates, which can be higher than corporate rates.

One of the most important tax provisions for small businesses, what's known as the Section 179 deduction, will shrink to $25,000 next year from $125,000 in 2012. The deduction, which applies to equipment purchases, was $500,000 in 2011. Congress can increase the deduction at any time, even after 2013 has begun. But for the time being, business owners can't count on getting a big break.

"It's a huge change for companies planning on making investments," McCracken says.

It's not known if Congress will extend the 2 percentage point payroll tax cut that workers have had for two years. If it doesn't, consumers will have less money in their paychecks to spend, and that is likely to affect retailers and any other small businesses that sell directly to the public.

Health care
Health care has been another source of uncertainty for small business owners. The new year will bring some, but probably not all, of the answers to questions about how the new health care law will affect them. Many will have to devote some time to understanding the law ? or hire someone to help them do it.

"They'll have to get their arms around the law, look at their options, learn more about the exchanges," says John Arensmeyer, CEO of Small Business Majority, a lobbying group.

Under the law, companies with 50 or more employees will be required to provide affordable health care insurance for their employees starting Jan. 1, 2014. During 2013, federal and state health insurance exchanges will be set up, and owners will be able to see how much it will cost them to buy insurance. As the year begins, however, many small business owners don't know whether their states will be creating exchanges, or whether they'll have to go into the national system ? and they don't know what that will mean for their costs.

For some owners, that information will help them decide whether they will buy insurance, or whether they'll decide it's cheaper to not provide coverage and just pay the government a $2,000-per-employee fine. For those who have close to 50 workers, they may decide to not hire more workers in order to remain outside the law's jurisdiction.

Lending
Don't look for the small business lending climate to get easier in 2013. Owners who are uneasy about the economy, taxes and health care aren't expected to significantly increase their borrowing, especially as many have been paying down debt since the recession. But even those who are ready to borrow are expected to find it's still hard to get a loan. Bankers are unlikely to be more liberal in their lending policies.

Depressed lending levels may be with us well beyond 2013, says James Schrager, a professor of entrepreneurship at the University of Chicago Booth School of Business.

The problem isn't just that banks are cautious about small business loans. Schrager notes that home equity loans, a traditional source of money for people starting or expanding a business, remain difficult to get, the result of the collapse in the mortgage market in 2008.

Small Business Majority's Arensmeyer is hopeful that a bill introduced in Congress this year to allow credit unions to make more loans to small businesses will get more traction when the new Congress convenes. That bill would more than double the amount of total assets that credit unions can use to lend to small businesses to 27.5 percent.

But he's also not expecting major changes in lending next year.

"Sadly, I don't think we're going to increase our access to capital overnight," he says.

Onshoring
A trend that's expected to gain speed in 2013 is what's calling onshoring. That's the term for manufacturing that had been done overseas, and that's now taking place back in U.S. factories. Apple Inc. earlier this month said it would move production of some of its Mac computers to the U.S. from China next year ? but many small businesses have already been making the switch. While Apple is an example of a big company moving in this direction, the majority of U.S. manufacturers are small businesses.

There are several reasons behind the trend. As China becomes more of a middle-class country, wages for its workers are rising, and that is lessening some of the appeal of manufacturing there for U.S. companies, says Steven Kaplan, a professor of entrepreneurship and finance at Chicago's Booth School. Over the past two decades, it was the cheaper labor in China that prompted businesses of all sizes to have everything from computers to clothes to food ingredients made in China.

The rising cost of fuel, which has made transporting goods more expensive, is another factor in onshoring.

"Manufacturing in the U.S. is relatively more attractive than it has been in 20 years," Kaplan says.

Skilled worker shortage
While companies' caution has weighed on the job market, many company owners who actually want to hire say it's hard to find workers to fill some positions.

It's becoming more difficult to find people who have the skills they need, these owners say. Many new manufacturing jobs require high-tech skills. They include positions at factories where computers are used to create products like airplane parts and machinery. And some require several years of training, says Shane, the Case Western Reserve professor. For example: A company that makes metal molds which are in turn used to create automobile dashboards. That takes a machinist who knows cutting-edge processes to make the molds.

"You cannot take someone off the street to do it," Shane says.

Because of changing technology, owners are struggling to find qualified workers in 2013.

"There's a whole level of work that's going to require skills that weren't needed in traditional jobs," says Arensmeyer. He notes that community colleges around the country are offering courses that will help train workers to fill these jobs. But training takes time, and the demand for jobs may continue to outstrip the supply of qualified workers.

Zach Udko: The Top 10 Top 10 Lists of 2012

2012 was the year that was supposed to destroy us -- and it nearly did.

After surviving an unparalleled disaster that rocked us to our core (Lindsay Lohan in Liz & Dick), we pulled together as a nation of Thelmas and Louises, ready to boldly drive off the fiscal cliff together.

Now, as December draws to a close, our nation's critics and bloggers have once again generously given us their thoughts on the main cultural and newsworthy events of the year in the form of Top 10 lists. Since my earliest childhood, I have always been a sucker for these holiday treats, savoring the opportunity to measure the tastes of each scribe against my own.

These essential lists provide us with a sense of perspective, comfort and order. They fulfill a deeply spiritual need to make sense of our existences in the face of a cruel and capricious universe. As the Italian novelist Umberto Eco explains, "It's a way of escaping thoughts about death. We like lists because we don't want to die."

And so, in recognition of their enduring value, instead of offering my own Top 10 films, plays or books, I present to you my Top 10 Top 10 Lists of 2012. I hope that a close inspection of these items (varied in content, unparalleled in exuberance) helps you grapple with the incomprehensible as we begin to face new mysteries in 2013. Without further ado, these are the lists that fascinated and enlightened me this year:

10. Top 10 Most Disgusting Objects Allegedly Found in Fast Food (TheWeek.com)

The list that inspired me to learn how to cook.

9. (Tie) Top 10 Best Dressed at the Golden Globes (Vogue) and
Top 10 Worst Dressed at the 2012 Teen Choice Awards (Wetpaint.com)

5. Top 10 Weirdest Animal Stories of 2012 (National Geographic)

We finally found a species that urinates out of its mouth. Thank you, National Geographic!

4. Top 10 Items From Oprah's New 'Favorite Things' List That We Actually Love (Business Insider)

2. This list

Unfortunately, I missed out on the number one spot, which clearly had to go to...

2013 Smart Guide: Next-generation video games

Read more: "2013 Smart Guide: 10 ideas that will shape the year"

Bigger, better and far more immersive. With Nintendo's launch of the Wii U in November and Microsoft and Sony both expected to pitch in with successors to the Xbox 360 and PS3 in time for next Christmas, 2013 will be a huge year for console gaming.

Leaked details have sparked excited chatter about 3D gaming in 1080p HD and Ultra HD (which has four times the pixels of 1080p), and augmented-reality play in your living room. Nintendo, which was the first to introduce motion-controlled gaming last time round, has continued its pioneering streak and brought us two-screen play, by adding a touchscreen to its control pads.

The gaming field is getting crowded. Since the launch of the last generation of consoles in 2005-2006, games have begun streaming from the cloud, coming to TVs directly from PCs, even become available through a crowd-funded Android console.

Given the eclectic competition, it's no wonder the next crop of heavyweights are embracing novel designs. The idea of the Wii U's controller screen is to clear the TV screen of user-interface clutter and introduce multiplayer possibilities. Players in the room have different perspectives on the action depending on what is displayed on their controller, for example.

Sony and Microsoft are also embracing two-screen concepts. The next PlayStation will probably enhance its connection to the PS Vita, its portable cousin, allowing you to play a game on the move and then swap to the main console when you're home. Similarly, the next Xbox is expected to make Smartglass - an app that lets you control the console from a smartphone - more of a centrepiece.

If additional screens and eye-popping pixel counts aren't enough, perhaps we'll move beyond screens. Microsoft has filed a patent for projecting augmented-reality 3D images onto your living room walls. If that pans out, game worlds will burst out of the TV and into your home, with Kinect - or its successor - letting you control the action with simple gestures. Two screens might seem rather quaint when your whole living room is the control pad.
